I have only encountered this at EU based vendors. I have never encountered this at a US merchant.
Not from my experience. Many stores allow to set a different country as shipping address too (e.g. Amazon UK).
I'm buying my Apple devices from the US. I live in Europe. It's that irritating :)
Amazon isn't intentionally discriminating against your country, they don't have a choice.
