Sounds like a job for some dynamic DNS. That's exactly the sort of thing it's supposed to solve for.
Unfortunately this also requires a dynamic dns client and server if you have a residential IP address that can change.
You can use dynamic DNS if the IP address you're getting is a public one.
Assuming you have a dynamic ip this won't be possible without a cloud service.
The address is public, you can use a dynDNS easily. Relying on fixed addresses never works.

Non-static IP, all done via dns updates. Never had an issue.
Also, doesn't work if your service provider provides dynamic IPs.
I have a dynamic IP from my ISP at home, so that wouldn't work?
A dynamic dns would solve that.
