There is no contradiction. You can tell obese people that their condition will cause them various troubles that they wouldn't have otherwise. You don't have to resort to shaming them for that. Beauty is subjective.

The changes I can’t understand most involve calling people fat. Obesity is a fact and it should not be normalized. Any obese person put on a desert island and fed water and a limited caloric diet would lose weight, a scientific fact. Any attempt at normalizing obesity is the same as normalizing smoking cigarettes or alcoholism - harmful to all of humanity.
I think the root of it is recognition that mental health is as important as physical health, and that losing weight isn’t as easy as many people assume, and shouldn’t be done the way many people try - so actively shaming and criticizing fat people for being fat is of negative health utility overall.
That may be true for most people, however, there are many conditions that cause obesity outside of those two factors.
