It'd be valuable for you to complain to Amazon when that happens. They probably can't get feedback on a service like this except through complaints like that.

i feel like there's a part of this story that we're not hearing. given amazon's excellent customer service reputation (including my own experiences) it's hard for me to believe that this could happen without there being a reason.

Reminds me of how Amazon would only accept complaints about fraudulent and scammy listings if you'd fallen for the scam.
